Where, what, and how it went — a couple lines is plenty. Fill it in and it opens a pre-filled email; attach a photo if you've got one before you hit send.
This is a stopgap until the new forum's live — reports go straight to Jeff's inbox and he'll get them posted. Once the forum's up, this becomes a real "new thread" form.
